The steps of the lesson including the problem you will pose to students and an explanation of how the problem is relevant to your students.
The students will first review the results of their diet journals from the previous week’s lesson. They will then use the Calorie and Nutrient Content Reference Guide to analyze their dietary consumption habits, the students will then use these results to predict their long term health status over a period of twenty-five years. The students will create a powerpoint presentation detailing and summarizing the health status of this individual, highlighting the link between poor dietary habits sustained for twenty-fives and the resulting physical results. Their artistic creativity will be encouraged in the visual portrayal of the characters appearance at the end of the period of study.
How students will use social networking and/or online collaboration tools in the lesson.
Students will search the internet for long term health related issues associated with and resulting from an improper diet and an inactive lifestyle such as obesity, diabetes, heart disease, cancer and other chronic illnesses. The use of a wiki for each of the groups will allow students the opportunity to network with each other while they are in school or away from school.
How you will customize the activity for diverse learners in your classroom.
Students will remain in the same already teacher selected groups from the previous week, these groups were determined by my attempts to balance out the ability levels of the students as equally as possible.
How will you assess student learning as a result of this lesson. Student assessment will be based on the process that goes into the development and production of the product, and more importantly it will be based on the final product itself.
How you will manage this activity in your classroom.
Classroom management will be accomplished primarily by visual monitoring of student activity as I move from group to group, conferencing with the students as needed.

Again, this is a really wonderful way to bring the real world into the classroom. Are your students already proficient with wikis? If not, how do you plan to teach/model the process with them? What about students who don't have internet access at home?

This may have nothing to do with your lesson, but have you shown your students the movie Super Size Me? I showed that to my high school students a few years ago and the discussion and reactions were amazing! I can see this leading to some great online collaboration and social networking, I wish they had been more common 5 years ago when I showed it, oh well!
